The two usual causes here are the water table and salt air. Roots sitting in water through winter rot slowly, so the plant often looks fine the first year and fails in the second. Salt scorch shows on the exposed side first. Both are fixable by raising the root zone and changing the plant list, rather than by replacing the same shrub again.
On the low ground, often yes. Where the water table is close to the surface, lifting the root zone is what lets a bed establish at all. On higher North Delta lots it is usually a choice rather than a necessity.
